This certificate is the first issue Type-1, 2 or 3 for the U.S. Coast Guard. For issue in the 1900’s or 2000’s bearing no preprinted numbers in the year block under the citation area. This certificate is considered a novelty item, and is not intended for any form of identification. It is blank and ready to be filled in by whom ever needs a replacement document. It measures 8 ½ by 11 inches, and printed on high quality government card-stock paper with the USCG Achievement medallion in the top center of the document, bearing the straight Ribbon Banner. The certificate also bears the non-embossed seal of the United States Coast Guard. The seller will print, NAME, RANK, UNIT, DATES AND CITATION FREE OF CHARGE IF DATA IS PROVIDED AT THE TIME OF PAYMENT, VIA PAY-PAL NOTE TO SELLER OR BY MAIL IF PAYING BY MONEY-ORDER. Authorized by the Secretary of the Treasury on January 29, 1964, as the Secretary of the Treasury Commendation for Achievement, this medal was re-designated the Secretary of Transportation Commendation Medal on March 13, 1967. Awarded to all members of the Coast Guard, including reserves and members of other branches of the Armed Forces, when serving with Coast Guard units, for outstanding leadership, professional achievement and superior performance of duty in either peacetime or combat situations.
